Liability Waiver

WAIVER: AGREEMENT TO HOLD HARMLESS, WAIVER AND ASSUMPTION OF RISK I understand that participation in a dog training session is not without risk to myself, members of my family or guests who may attend, or my dog. I hereby waive and release Beyond the Leash K9 Training and its employees, officers, members, and agents from any and all liability of any nature, for illness, or damage which I or my dog may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dogs, or damage or injury from any other cause, and I expressly assume the risk of such damage or injury while attending any training sessions, or any other functions with Beyond the Leash K9 Training, or while on the grounds of Beyond the Leash K9 Training Center at the address above or other designated areas/addresses. It is further understood that I need to meet Beyond the Leash K9 Training's vaccination protocol for my dog/puppy to attend classes. I understand that should it be my choice to NOT vaccinate I accept all responsibilities that may arise from that decision. In consideration of and as inducement to the acceptance of my application for training at Beyond the Leash K9 Training, I hereby agree to indemnify and hold harmless Beyond the Leash K9 Training, its employees, officers, members, and agents from any and all claims, or claims by any member of my family or any other person accompanying me to any training session or function with Beyond the Leash K9 Training, or while on the training grounds of Beyond the Leash K9 Training Center or other designated areas/addresses.

Media Release

Media Release I grant Beyond the Leash K9 Training permission to take photographs of myself and/or my pet, and to publish those photographs for any lawful purpose, including, but not limited to, their website, social media accounts, and promotional materials, either digital or in print, in perpetuity. I also grant permission to use my name and/or my pet’s name. I authorize Beyond the Leash K9 Training to edit, alter, share, remix, tweak, build upon or in any way alter the photograph(s) mentioned above. I also waive any rights of privacy or compensation associated with the use of my or my pet’s image(s) and name(s) for the personal or commercial purposes outlined above.
